This can be a copy of your till receipt, delivery note, UK gift receipt or, if the gadget is a mobile phone, confirmation from your Network Provider that the mobile phone has been used by you. Evidence of ownership must include purchase date, make / model and IMEI / serial number.

This must match the post code associated with your insurance policy. Please include a space between the first and last three characters - .e.g. OX4 2ER.

Please confirm the manufacturer of your gadget.

Please confirm the specific model of gadget, including capacity.

IMEI number

Your IMEI or serial number will be listed on your policy schedule and is also on your evidence of ownership. Otherwise you can type *#06# on your phone's keypad to see your IMEI number.

Claim Details

You will need to be able to fully explain the circumstances in which your device was lost, stolen or damaged. Please confirm where you were, what happened and the time of the incident.

Evidence of Ownership

Please provide evidence of ownership for the item that you are claiming for.
